When to use it
Send it right after: after support, after delivery, after the visit. Satisfaction measured a week later isn't satisfaction any more, it's the story someone tells themselves about it.
Three things that help
- Stars first: it's the question people answer without thinking, and someone who has answered one tends to finish.
- The matrix is there to tell you *where* you're weak: a high overall score with «price» at two says something the score alone doesn't.
- Ask for the email last, optional, and only to follow up: putting it first makes the whole thing read as lead collection.
